Can subcision with fillers completely remove acne scars?

Doctor's Answers 1

Hi there! Don't worry, there are many proven ways to improve your acne scars and skin complexion!

Subcision with fillers like hyaluronic acid or PDRN (Rejuran) can certainly help to improve your acne scars dramatically with visible improvements every treatment session.

Usually at least 3-5 treatment sessions are required for good results, especially if you have deeper or wide area of scarring.

To get even better results for acne scar removal, I usually combine subcision + fillers with energy-based treatments like lasers (CO2 Fractional Laser/Pico Fractional Lasers) and radio-frequency microneedling (INFINI, Intracel) to enhance collagen stimulation and scar remodelling.

However, do note that not every type of acne scar will respond as well to subcision and filler. Rolling atrophic scars tend to respond well to subcision.

For other types of acne scars like ice pick acne scars, other treatments like TCA cross deep chemical peel might also be more suitable.

The key to good acne scar improvement is to use the appropriate treatment for each type of acne scar.

How effective are the various types of chemical peels for acne scars?

Medical-grade Chemical Peels are highly effective treatments for Acne Scars. It is proven that TCA Cross 70% alone can give you good to excellent improvement (> 50% improvement) of atrophic acne scars, especially severe boxcar scars. I routinely perform TCA Cross for ice pick scars and deep box acne scars. TCA Cross, which stands for Trichloroacetic acid, is a medium to deep chemical peel. There is a wide range of concentrations of TCA cross (from 35% to 100%). I usually combine this with subcision, Lasers and Rejuran injections for enhanced results in treating ice-pick scars.

How much improvement can I expect after subcision for my acne scars? (photo)

Acne leave behind mainly ice-pick or box-car scars. The base of each scar is held down by connective scar tissue. My preferred method is to use a combination of laser treatment and Rejuran Injections to improve the overall texture of the skin. The PicoWay has an attachment called the RESOLVE handpiece. When the laser enters the skin, it creates areas of cavitation under the skin called Laser Induced Optical Breakdowns. This helps to disrupt the connective tissue holding the base of the scar down.
