What documents or medical information should I prepare if I am planning to switch orthodontists?

As orthodontic treatment takes time to complete, typically 1.5 to 2 years, it is not uncommon for some patients to have to switch orthodontists due to different life circumstances. These are some medical information that I feel will greatly benefit your new orthodontist:

  1. Pre-treatment or starting Xrays (Orthopantogram and lateral cephalogram)
  2. Pre-treatment photos of your teeth and facial photos
  3. Study models (if available) or photos of study models, some clinics may have a digital version of the study model if they routinely use an intra-oral scanner
  4. Your orthodontist's treatment plan (e.g if teeth were extracted and which teeth)
  5. Brand and prescription of the braces used as well as the current set of wires
  6. If you are currently on clear aligner treatment, the contact of the laboratory that your orthodontist is working with. If it is Invisalign, your new orthodontist can request for a transfer of the ClinCheck plan
