What causes swelling 1 week post-op under eyes after a malar filler with voluma?

Well, from the picture, it appears that the left periorbital region has some redness and swelling.

  • Post fillers, there may be a reactive swelling due to the "absorption" of fluids by the filler, this is benign and tends to subside over a few days.

  • If there is an obvious lump felt, then it may be a superficial placement of the filler which made the initial swelling a little more pronounced. Do see your doctor to address this problem, the use of hyaluronidase (melting) can be considered.

  • The main concern would of course be infection. As long as reasonable cleaning with alcohol AND chlorhexidine was done, the risk is minimal. This is most likely if the area of swelling is also warm and mildly painful.

Although there appear to be differences in the size of the eye (the vertical aperture), as well as the height of the creases, it’s important to assess these with the eyes in a neutral gaze (head not tilted and looking straight ahead). If there are differences in the size of the eye, this will need to be corrected by adjusting the levator muscle in the upper eyelid (ptosis surgery). The height of the eyelid crease can be adjusted by correct placement of the skin incision, as well as by removal of small amounts of skin.
