Hi,

Fractional CO2 laser is the best modality for the treatment of boxcar scars. Ice-pick scars may also be treated with fractional laser although treatment response is suboptimal.

TCA cross may be used only to treat ice-pick scars. It should not be used to treat boxcar scars as scar widening and worsening may occur. TCA cross remains a difficult treatment modality with a narrow margin of treatment safety. Alternative treatments of ice-pick scars include punch excision.

Hmm, this is an interesting question. But my take on it is:

  • **ICE PICK
    **I would definitely first start with TCA Cross as it would reduce the depth of the scars significantly, before finishing up the last bit with fractional lasers. I had some pretty happy patients with this methodology. I explained it to them that its essentially, "scarring the scar together"

  • BOXCAR
    I would actually focus more on fractional laser OR microneedling as the modalities of choice. This would generally improve the texture of the skin as well. Adding on subcision for appropriate scars would be optimal as well.

Are there any topical products that can help with pitted acne scars, or must I opt for fractional laser treatment? (photo)

Hello Bemme Thank you for your question. You can try topical retinoids (retin A/ differin) which will help with mild acne scars, acne control, skin texture and oily skin. Use it at night for 1 to 2 months, and you should be able to see some results. However it is not enough to shrink your pores. A fractional resurfacing laser or microneedling fractional radiofrequency treatment can be done in combination for better results and more severe acne scars and uneven skin texture. It will also improve small to medium sized pores. Speak to a doctor to find out more. Hope this helps!
