From the photo, you may not have milia on the cheeks but may have multiple closed macrocomedones. Macrocomedones are facial closed comedones. Typically, it is larger than 2-3mm in diameter.

Treatment of macrocomedones can be very challenging. Treatment options include:

  • topical retinoids,
  • superficial chemical peeling,
  • light electrocautery and
  • oral isotretinoin.

While oral isotretinoin tends to be very effective, initial flare of acne (which may be severe) are common and has to be managed carefully.

Thus, it is very important that you see your preferred aesthetic doctor for a thorough evaluation and discussion on the best treatment option for your condition.

It appears that you have a milium (a single milia). Milia is the medical term for dilated cystic glands which are found just under the skin surface. They are usually 1 to 2 millimetres in size. It is caused by trapped keratin (dead skin cells) that clog up the swear ducts. This condition is rather common in the lower eyelid and can also affect the upper eyelid, nose cheek and chin. Whilst milia may look like whiteheads, they can’t be popped by squeezing. My preferred treatment for milia that’s stubborn would be to use a Smartxide CO2 laser.
