From the photo, you may not have milia on the cheeks but may have multiple closed macrocomedones. Macrocomedones are facial closed comedones. Typically, it is larger than 2-3mm in diameter.
Treatment of macrocomedones can be very challenging. Treatment options include:
- topical retinoids,
- superficial chemical peeling,
- light electrocautery and
- oral isotretinoin.
While oral isotretinoin tends to be very effective, initial flare of acne (which may be severe) are common and has to be managed carefully.
