Hi!

From the photo, it appears that you have acne scars rather than large pores. Physical examination in person would allow us to be more certain and advise better on the approach.

What are effective treatments for large pores on dark ethnic skin if chemical peels and fractional C02 laser does not work?

Dark ethnic skin has more melanin content and is more prone to post inflammatory hyperpigmentation (PIH), which can be persistent.

Chemical peels, with the exception of CROSS technique, are not really what I would recommend for scarring.

Fractional CO2 laser and other fractional lasers deposit energy and heat damage at the skin surface before penetrating deeper. Depending on technical parameters such as the wavelength, spot size, pulse duration and coverage of the laser, this collateral damage to the skin surface can stimulate excess melanin production as a protective response - PIH.

For dark ethnic skin, I would recommend fractional microneedle radiofrequency or fractional picosecond laser over fractional lasers or chemical peels.

  • Fractional microneedle radiofrequency such as Explore, Infini, etc use microneedles to bypass the skin surface, only depositing energy in the deep skin layers. This allows aggressive treatment while sparing the skin surface, reducing the downtime and chances of PIH greatly.
  • Fractional picosecond lasers produce cold photomechanical damage deep in the skin. The lack of heat damage reduces the chances of PIH.

Between the two, fractional microneedle radiofrequency seems to produce more obvious results while fractional picosecond laser has shorter downtime.

From the photo, you appear to have mild acne scars rather than open pores.

Fractional CO2 laser should work. Effectiveness may be enhanced with more aggressive treatment parameters albeit at the expense of perhaps more unwanted side-effects such as PIH.

Alternatives to consider are Fractional microneedle RF and fractional picosecond laser.

It is best you discuss with your preferred aesthetic doctor your expectations and ability to accept downtime for him/her to help devise an individualized treatment plan for you.

Darker-skinned patients can consider the following treatments:

  • Laser treatments
  • Ultracel Q+
  • HIFU
  • Fractional radiofrequency devices

Laser treatments, Ultracel Q+, HIFU and the use of fractional radiofrequency devices can be performed safely on darker-skinned patients to reduce the pore size. The settings applied to the devices are lower than those used for patients with fairer skin.

HIFU and Radiofrequency treatment is not colour-sensitive and so it can be performed on anyone, however with slightly modified lower settings for patients with darker compared to fairer skin.

Long wavelength non-invasive laser treatments performed at lower settings can reduce the pore size but must be performed with care.

Hi,Your enlarged pores are best treated by a combination of topical retinoids, chemical peeling and energy-based fractional procedures (e. g. fractional CO2 laser, fractional microneedle RF, fractionated picosecond laser). Hi,For persistent acne, you should see your doctor to evaluate for an underlying cause such as hormonal imbalance and to assess for any controllable risk factors such as diet, stress, comedogenic skincare products. Thereafter, appropriate treatment can be offered based on your age, gender, response to previous treatments, suitability and cost considerations.
