From your question can I presume that your shoulder pathology came about from an injury? If so, and if it is relatively recent, it certainly would be worth seeking a surgical opinion and discussing the possibility of a repair. Often, my surgical colleagues report, that you cannot be 100% sure of the injury or what can be done until during the operation.

If the injury was some time ago, if there is a degenerative element and depending on your function, it might be worth considering more conservative measures in the first instance. This might include physiotherapy, ultrasound guided treatments or adjuvant therapies such as acupuncture for pain.

I presume you have had a MRI scan of the shoulder? If this was done with a surgeon or sports physician, then it would be worth discussing further with them.
