What are the tests available in Singapore to check for Hereditary diffuse gastric cancer (HDGC) if my family has a history of gastric cancer? How much do they cost?

Gastric cancer is mostly sporadic and related to environmental factors although approximately 5-10% are genetically linked. Common environmental factors associated with gastric cancer include

  • Helicobacter pylori infection
  • Smoking
  • Regular intake of preserved and salted foods
  • Obesity

If you are particularly concerned about HDGC, your best bet is a genetic mutation test. This is a specialised process involving genetic counselling and blood sample testing, possibly including or requiring a sample from the index case for a more definitive result. I suggest a visit to the following website for more information on the exact nature and cost of genetic testing: https://www.nccs.com.sg/patient-care/specialties-services/cancer-genetics-service

Please be informed that the preventive treatment of HDGC, if found to carry the defective gene, is surgical removal of the entire stomach before cancer develops. There is no standard recommendation regarding the best time for the surgery although some experts suggest around 30 years of age or at least 5 years earlier than the index case. So please consider carefully on the merits and what is at stake, including the psychological impact on you and your family before you embark on taking such a test.

Are there any alternatives to chemotherapy for breast cancer after a mastectomy?

Chemotherapy is an adjuvant treatment modalty aimed to reduce the overall risk of cancer recurrence. The benefit and need for chemotherapy can vary from patient to patient depending on the stage of cancer, cancer biology, patient baseline health status and relative age of the patient. Stage 3 breast cancers will generally benefit from chemotherapy and omitting it may lead to a high chance of recurrence. Depending on tumour biology and one of which is what we call the receptor status of the tumour ,will dictate if the patient is suitable for endocrine treatment (tablets).

Could persistent burping and belching pose a serious health risk?

Belching or burping is a normal body response to get rid of excessive gaseous material in the upper digestive tract. This works somewhat similar to the pressure-relief-valve found on pressure cookers to let go of excessive pressure build-up. In general, belching helps to reduce discomfort from gaseous distension. However, if this happens too frequently, it can result in disturbing symptoms such as heartburn, chest pain, painful or difficult swallowing, complications from acid burns might have developed in your oesophagus.
