Acne involving the scalp and back of neck is one pattern of acne that is well described.

The reasons include oily skin/scalp, genetic and personal predisposition.

More importantly is appropriate treatment (refer Acne section) should be given early as acne on the back of scar may cause keloid scarring and acne of the scalp may cause scalp scarring and localized permanent hair loss.

Dr Justin says it well. Treat the acne first. IF not its an absolute waste of time chasing after the scars when new ones keep appearing. More often than not, products can play a large part in acne. What are the side effects of Spironolactone for acne?

Hi,Spirolonolactone can be used for acne treatment but this is an off-label indication. It is mainly used as a diuretic and anti-hypertensive medication. It has anti-androgen effects which is helpful for treatment of women with hormonal acne. Side effects of spirolonactone include elevation of the potassium levels and this may need to be monitored. It can also affect fetal development and it is important to avoid pregnancy while on the treatment. Other potential side effects include dizziness, breast discomfort and irregular menses.
