The elevation of the liver enzymes can suggest a few possibilities. The commonest cause of liver enzyme elevation of this magnitude is fatty liver which can be picked up most times by doing a simple ultrasound scan of her liver.

Patients with fatty liver may also have concurrent diabetes, hypertension or elevated cholesterol level. They tend to be slightly overweight as well.

Other causes include undiagnosed viral hepatitis infection, mostly Hepatitis B or C. You can scan through her results to see if such causes have already been excluded during her health screening. Besides that, health supplements and medicine may cause liver function abnormalities. In such cases, the liver enzymes usually normalise when the culprit medicine or supplement is stopped.

May I advise your mom to pay her doctor a visit say a few weeks later to see if the enzyme levels are still high. If so, she should consider seeing a specialist to get her problem sorted out.

Is it worth having a colonoscopy as part of a thorough health screening check in Singapore?

If you are above the age of 45, or have a positive family history, it is important to consider a colonoscopy as part of your routine health screening. This is because colorectal cancer is the number 1 cancer in Singapore and more often than not begins as a polyp. Polyps are small and have no symptoms. The only way to remove polyps is through a colonoscopy.

When are elevated liver enzymes a cause for concern?

Isolated elevation in ALP level, especially if marginal and transient in nature without any elevation in the other liver enzymes and serum bilirubin level usually does not signify a serious medical problem. Some teenagers and young adults may have marginally elevated ALP levels due to more active bone metabolism. This is considered a normal variant. Drugs and fatty liver can also cause ALP elevation without a concurrently higher GGT level.
