When is the best time to take a pregnancy test for the most accurate results?

There are 2 main types of pregnancy tests:

  • a urine test kit which can be performed in the convenience and privacy of your own home or in the clinic
  • a blood test that must be performed in the clinic.

Both tests work by measuring if there is a hormone called Human Chorionic Gonadotrophin (hCG) which is produced after a fertilised egg is attached to the womb.

Generally, I will recommend my patients to take the test if they missed their menses because the hCG should be detectable by then for both tests.

Both tests work by measuring if there is a hormone called Human Chorionic Gonadotrophin (hCG) which is produced after a fertilised egg is attached to the womb.

The urine test kit is what I would call a “qualitative” test kit, it gives you a “Yes” or “No” answer and it works once the hCG reaches a certain level. The test kits available in the pharmacies are generally quite accurate and would be sensitive enough to pick up a pregnancy if you missed your menses by a week and suspect you may be pregnant.

The blood test is a “quantitative” test kit and it measures the amount of hCG in the blood. A level of more than 25 mIU/ml is considered positive for pregnancy. It is, therefore, more sensitive in a way and can pick up pregnancies even earlier. As such, I use this test if my patient is very anxious and wants to pick up the pregnancy earlier or if my patient has an irregular menstrual history and does not know if she has missed her menses.

For a young lady with a history of irregular menses, we will always consider polycystic ovaries as a possible cause. Other possible causes include severe stress, hormonal disorders such as thyroid problems, prolactin producing tumours (prolactin is a hormone necessary for production of breast milk), sudden weight fluctuations, vigorous intensity of exercise, or even a deficient production of the female hormones. Of course, in someone who is sexually active, it is important to exclude pregnancy. The urine pregnancy tests kits are quite sensitive and hence the result is reliable enough.

Could I still be pregnant despite a negative pregnancy test if I’m experiencing brown discharge?

If you suspect there is a high chance of pregnancy, I would suggest repeating the urine pregnancy test in a week’s time. If it still negative then it is unlikely you are pregnant. Irregular bleeding like what you describe is not uncommon in PCOS even in the absence of pregnancy. I would recommend using contraception if you are sexually active. In addition to preventing pregnancy, contraception like the oral contraceptive pill (OCP) will make your periods regular and light, and prevent endometrial hyperplasia (build up of womb lining) which is very common in PCOS.
