Hi there,

The answer is yes, there are alternatives to Accutane for oily skin. Systemic isotretinoin (Accutane, Oratane) is very effective for reducing seborrhoea (oily skin).

Research has shows that 1 year after the completion of isotretinoin therapy, sebum excretion rates have been found to remain significantly suppressed for most individuals.

However, for various reasons such as potential side effects of systemic isotretinoin, some people may be unsuitable to take this medication.

Your alternative medications are:

1. Topical retinoids

2. Spironolactone (for women only)

3. Oral contraceptive pills (for women only)

Some procedures may be suitable too; these include chemical peels, lasers and botulinum toxin. However, do speak to your doctor who will be able to advise more on these procedures first.

Best regards,

Dr. Joanna Chan

How safe is low dose Accutane/Isotretinoin in the long term?

It is good to hear that you have achieved good control of acne with isotretinoin. The low dose that you are taking now will reduce sebum (oil) production and maintain your condition. From research studies done on adults, low dose isotretinoin showed no side effects to the body when the researchers did an extensive panel of blood tests on patients who took isotretinoin 20mg twice a week for 3 years. Your dose is even lower than this so it is safe, provided that you do not have habits such as excessive alcohol intake, as that will stress your liver.

Is retacnyl, laser treatments or chemical peels more effective for eliminating acne scars?

Hi Aaron – I completely agree with whatever your doctor has done. I had asked about accutane but my doctor was hesitant in prescribing to me due to my history with depression. I’d err on the side of caution as well, as depression, although rare, is one of the side effects of accutane. Moreover, you described your acne as mild so accutane would be an overkill. My dermatologist said that the retacnyl will help to clear my acne(very few) and my dark spots and mild acne scars. Would it actually help to eliminate my acne scars?
