Priapism or persistent and painful erection is a urology emergency. It must be attended by a urologist immediately and treatment would depend on the duration and severity of the condition.

Medications to relax blood vessels in the penis is given and this can be in the form of a pill or injection directly into the penis. Excess blood in the penis may be removed with a needle in more serious conditions.

It could be prostatic fluid as mentioned by Dr Winston, or possible infection with Candida, chlamydia, gonorrhoea or herpes simplex virus. You will need a full STD workup or urine and blood tests to ascertain the cause.
