What are possible causes for gradually progressing, sharp pain in lower abdomen, radiating to lower back and groin?

Any abdominal pain should have a through exmaination and imaging to exclude problems inside the tummy that could be the cause of pain.

The character of pain gives an indication of source as well. If it is dull, intermittent and gnawing, this is likely arising from inside the tummy and a CT scan should be able tk discern the cause.

If the pain is sharp and movement related; if there is a precipitating accident or trauma, this could point to a muscular source.

Pain that is associated with numbness, electric shock, cold or burning sensation would indicate that neuropathic pain is likely.

In this area, such possible causes could be arising from ilioinguinal and iliohypogastric, or genitofemoral nerves.
